PRENATAL DIAGNOSIS OF FRONTO-ETHMOIDAL ENCEPHALOCELE : A CASE REPORT

Introduction: Neural tube defects affecting the cranial vault are termed encephaloceles. The incidence of encephalocele is one to four in 10.000 live births. The vast majority 75 to 80 % of encephaloceles involve the occipital area,%15 the frontal areaand the remainder the parietal area (1,2). We present a case with fronto-ethmoidal encephalocele enlarging during pregnancy Case: here we reported a fronto-ethmoidal encephalocele case enlarging gradually.after amniocentesis and counselling the family opted termination of pregnancy. The diagnosis was confirmed postnatally. Discussion: Anterior encephalocele is a rare condition with a reported incidence between 1:35 000 and 1:40.000 live birth . The incidence is higher in southeast Asian countries (3). Anterior encephaloceles are divided into two types sinsipital and basal defects. An encephalocele is a type of malformation in which there are calvarial aned dural defects with extracranial herniation of leptomeninges and sometimes of brain and cerebrospinal fluid . anterior encephaloceles probably result from defective development of prosencephalic neural crest tissue (4). It is important to show the bone defect because because when the bone defect is absent the differantial diagnosis included dacrycystocele, hemangioma, lipoma, epidermoid cyst ,teratoma can change the pregnancy management. The prognosis of encephalocele is worse than the others.the diameter of the encephalocele sac should be checked regularly because enlarging sacs have a worse prognosis. Conclusion: Prognosis of fetuses with encephalocele is different from fetus with hemangioma, dacryocystocele, lipoma, epidermoid cyst and teratoma .so differantial diagnosis is important. If couple wish to continue pregnancy; regularly checking encephalocele sac diameter is important for counselling.
